Comprehensive Major Medical Plan
(Humana Employers Health)

Regular, full-time. First day of the month following 30 days of employment.

Fully-insured comprehensive major medical with enhanced preventive care benefits.  PPO.  $5,000,000 lifetime maximum.  Precertification required.  Pre-existing conditions 6/12.  Dependent ages 19/25.

In-Network:

Annual Deductible: $500 individual. $1,500 family.

Office Visit Copay: $15.

Coinsurance:  90% after the annual deductible, including office therapy, office surgery, inpatient care, outpatient hospitalization, emergency care, maternity, and home health care.  Prenatal care:  100% to $200.  Preventive Care:  100% to $200.

Emergency Care: $75 copay, then 90% after deductible.

Out of Pocket Limit:  $1,000 individual, $2,000 family.

Out-of-Network:

Annual Deductible:  $500 individual.  $1,500 family.

Coinsurance:  70% after the annual deductible, including office visits, office therapy, office surgery, inpatient care, outpatient hospitalization, emergency care, maternity, and home health care.  Preventive Care:  100% to $200.

Emergency Care: $75 copay, then 70%

Out of Pocket Limit:  $3,500 individual, $10,500 family.

Prescriptions:

$10 generic formulary copay.  $20 brand name formulary copay.  $35 non-formulary co-pay.  Mail order available.

Indemnity Dental Plan
(Humana Employers Health)

Regular, full-time. First day of the month following 30 days of employment.

Dependent ages 19/25.

Annual Maximum Benefit: $3,000

Annual Deductible: $50/$150.

Preventive: 85% (cleaning, fluoride, x-ray) after deductible.

Routine: 85% (filling, extraction) after deductible.

Major: 50% (inlay, bridge, crown, dentures) after deductible.

Orthodontics: 50% to $1,500 lifetime maximum.

CDG pays 100% of the cost for employee coverage and 50% of the cost of dependent coverage.

Group Term Life and AD&D Coverage

Regular, full-time. First day of the month following 30 days of employment.

Life: $50,000, reduced to 25% at age 70.

AD&D: $50,000 to age 70.

No cost to employee; premiums are paid by CDG.

Supplemental Term Life
(Hartford)

Regular, full-time. First day of the month following 30 days of employment.

1, 2, or 3 times earnings to a maximum of $500,000. Subject to evidence of good health for amounts over $40,000.

Rates are based on employee's age and earnings.

Supplemental Spousal Term Life
(Hartford)

Regular, full-time. First day of the month following 30 days of employment.

Increments of $10,000 limited to the lesser of $50,000 or 50% of employee's supplemental term life. Subject to evidence of good health for amounts over $20,000.

Rates are based on spouse's age and the amount of coverage.

Supplemental Child(ren) Term Life
(Hartford)

Regular, full-time. First day of the month following 30 days of employment.

Ages 6 months to 19 years (23 if full-time student). $1,000 increments to $10,000.

Rates are based on the amount of coverage.

Long Term Disability
(Hartford)

Regular, full-time. First day of the month following 30 days of employment.

Elimination Period: 90 days.

Benefits: 60% of monthly base wages/commissions to a maximum of $6,000 per month, minimum $100 per month, payable to age 65.

Definition of Disability: Own occupation.

Survivors Benefits: Lump sum payment equal to 3 times the participant's monthly benefit.

Benefit Limits: 24-month limit on mental/nervous benefits.

No cost to employee; premiums are paid by CDG.

Supplemental Long Term Disability
(Hartford)

Regular, full-time. First day of the month following 30 days of employment.

Buys up the employer-provided long-term disability to 70% of monthly base wages/commissions to a maximum of $10,000 per month.

Rates are based on employee's earnings.

Flexible Spending Accounts
(Precision Administrators)

Regular, full-time. First day of the month following 30 days of employment.

$5,000 Medical Reimbursement Account and $5,000 Dependent Care Reimbursement Account.

Administrative expenses are paid by CDG.

401(k) Plan
(ReliaStar)

There are no age or service requirements. Participation may begin on the first day of any calendar quarter next following the date of hire. Salary reduction may begin on the first day of any payroll period.

Salary Reduction Amounts: 0-15% of W-2 compensation.

Vesting: 0% with less than 5 years of service. 100% with 5 or more years of service.

Loans: Permitted. Loan amount available is minimum of $1000, maximum of one-half vested interest to $50,000.

Funds: 9: Bonds/Fixed Income: Oppenheimer Strategic Income Fund ReliaStar Stable Value Managed Account. Employee Stock Ownership Plan (ESOP)

Jan. 1 or July 1 following completion of 1,000 hours of service, age 21

Employer Contributions: Annual discretionary.

Vesting: 100% after 5 years.

Transfers/Rollovers: Not permitted.

Hardship Withdrawals: Not permitted.

Paid Time Off

Regular, full-time. First day of the month following date of hire.

Purpose: vacation, sick leave, disability, jury duty, and personal pursuits.

Less Than 5 Years of Service: 1.25 days accrue per month = 15 days per year for consultants.  1.08 days accrue per month = 13 days per year for Home Office.

5 Years But Less Than 10: 1.67 days accrue per month = 20 days per year.  1.50 days accrue per month = 18 days per year for Home Office.

More Than 10 Years: 2.08 days accrue per month = 25 days per year.  1.92 days accrue per month 23 days per year for Home Office.

Maximum Accrual: 500 hours.

No cost to employee; provided by CDG.

Holidays

Regular full-time employees, first day of employment.

New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, the day after Thanksgiving Day, Christmas Day, and 1 floating holiday for employees active on or before June 30th

No cost to employee; provided by CDG.

Short-term Disability Benefits

Regular, full-time. Documentation required.

After 30 days, CDG pays 60% of base wages up to 60 days. Available PTO must be applied.

No cost to employee; provided by CDG.

Jury Duty

First day of employment.

PTO or unpaid days may be used. CDG continues health insurance through the end of the first full month of unpaid jury duty.

Family and Medical Leave of Absence

Regular, full-time. One year of service.

Unpaid leave may be granted for up to 12 weeks per 12-month period with 30 days advance request for childbirth; adoption; serious health conditions of the employee or the employee's child, spouse, or parent.

Personal Leave

Regular, full-time. One year of service.

Unpaid leave may be granted for up to 30 calendar days every two years for personal obligations.

Sabbatical Leave

Regular, full-time. One year of service and a satisfactory performance record.

Unpaid leave may be granted for at least 30 days but for no longer than 12 consecutive months to engage in intensive research or for a period of rest to prevent job burnout.

Bereavement Leave

Up to 3 paid days for death in immediate family.

Military Leave

Unpaid leave will be granted to employees called to training or active duty with the U.S. armed forces. PTO may be applied.

Learning Credits

Regular, full-time.  Consultants only.  First day of the month following one year of employment.

Eligible consultants are awarded annual credits to apply to the job-related learning or training of their choosing.  25 credits in years 2-5, 35 credits in years 6-10, 45 credits in years 11+.

Professional Memberships

Regular, full-time. Certified. First day of employment.

One annual professional association membership and costs associated with CE credits for employees maintaining certain professional certifications.
